As for the 4x4s, they are not that bad, but you have to use your own judgement--it's a very "hands-on" trip, periods of jostling where you have to work to hang on. But it's slow-mo, not break-neck speed.
And since you're sitting sideways in the back of a jeep, motion sickness is another issue--if I go on one of these again, I will definitely pop a bonamine pill first.
The 4x4 portion of the river trip on Raiatea was quite tame, as I remember. The one on Bora Bora was the worst, jostling-wise.
