Agreed: the form was poorly designed and a major pain to fill out. I think I did it three times! But, I, too, used Travisa Outsourcing.. here in NYC. Dropped the stuff off Monday morning. Picked up the Visas on Tuesday afternoon. At least, that part went as per plan!
